Students entering 6th-8th Grade (and current 8th graders) are invited to join us on our Summer Campout!
Thursday - Sunday, June 8-11th
Sleeping Bear Dunes, Michigan
Cost is $100
Our theme is "Wandering in the Wilderness" As we wander through the wilderness of Michigan we will see how we can relate to the Israelite and also see God's love for His people.
We will be sleeping in tents under the stars (there is a bathroom and water source), enjoying the great outdoors, jumping off and climbing giant sand dunes, campfires, smore's, Lake Michigan, and SO MUCH MORE!!!
PARENT/GUARDIAN BE ADVISED
Upon registration you agree to the below medical and liability statement.
MEDICAL & LIABILITY RELEASE STATEMENT:
I understand that in the event medical intervention is needed, every attempt will be made to contact immediately the emergency contact person listed on this form. In the event I cannot be reached in an emergency during the activity dates shown on this form, I hereby give my permission to the physician and or dentist selected by the activity leader to hospitalize, secure medical treatment and/or order an injection, anesthesia, or surgery for my child as deemed necessary.
I (we) represent that we are the parent(s) or the legal guardian(s) of the above named participant and have the right and the authority to sign this release or liability. We hereby release absolve, indemnify and hold harmless Headwaters Church Inc. and its staff, elders, members, volunteers, and those persons transporting our child in any of the activities of Headwaters Church (all of whom are referred to as “released parties”) from any and all losses, damages, injuries, claims or causes of action in any way arising out of or in conjunction with the participation of the above named child in any activity of Headwaters Church. I (we) hereby release, absolve, indemnify, and hold harmless the released parties from any and all acts, including, but not limited to, the negligence of any of the released parties and hereby waive and release any and all claims of any kind or nature whatsoever against the released parties.
